CMS Proposed Rule: Medicare Part D cost-sharing, provider directories, and PBM accountability changes

CMS is proposing rules to implement three provisions of the Consolidated Appropriations Act, 2026: adjustments to Medicare Part D cost-sharing reductions for low-income individuals, requirements for more accurate health provider directory listings, and new pharmacy benefit manager (PBM) accountability standards. These changes primarily affect Medicare Part D plans, pharmacies, and PBMs. SUD treatment programs that bill Medicare for services or that help clients navigate Part D drug coverage may be indirectly affected by cost-sharing and formulary access changes.
